• Confirm if Vehicles have Cameras and or Telematics installed (if yes, MFG)
• Loss Control and or Safety Programs in place (if yes, copies if available)
• Copy of Expiring Policy/DEC Page and Rating Pages (if available)
• Target Premium
• Acord 137/State
• Completed Vehicle Schedule
• Completed Driver Schedule
• Driver MVR’s (if available)
